Episode Summary

Learn why rotation-related back pain often starts with limited hip motion and excess movement in the lumbar spine, and how signs like TFL dominance, pelvic asymmetry, and daily movement habits contribute. Discover simple, functional strategies to improve hip rotation, stabilize the pelvis, and reduce pain in walking, exercise, and sport.

Rotation syndrome is explained as a pattern in which movement that should occur at the hips is instead taken from the lumbar spine. This is often linked to insufficient lumbopelvic control, overactivity in hip muscles such as the TFL, and weaker abdominal control, especially from the external obliques. Common findings include a wide pelvis with a narrower trunk, possible leg length differences, structural femoral torsion, and pain during both flexion and extension, often with some lateral flexion. Assessment focuses on how rotation appears in walking, standing, and functional tasks, and whether symptoms improve when lumbar movement is reduced. Signs such as pelvic asymmetry, iliac crest height differences, and persistent TFL activity are highlighted. Treatment aims to limit lumbar rotation and restore more rotation through the hips. Practical strategies include keeping the pelvis steadier during walking, improving abdominal control, using leg-lowering exercises, stretching the TFL, and adjusting sport-specific positions such as foot placement during a golf swing. Everyday movement habits are treated as the main priority, with exercises used to reinforce those changes.
