Episode Summary

Explore a practical, less threatening way to understand lumbar spine movement by distinguishing between guarding and directional sensitivity, while keeping psychosocial factors in view. Learners will see how small movement changes, hip retraining, and symptom modification can reduce pain and build confidence in everyday tasks.

Lumbar spine movement patterns are presented as potentially useful when a clear pattern is obvious and changing that pattern reduces symptoms. The emphasis is not on labeling movement as “faulty,” but on recognizing sensitivity, creating new movement options, and remembering that pain is also influenced by fear, stress, poor sleep, guarding, and reduced activity. Two broad patient profiles are outlined. Type 1 involves guarding, tension, compression, and fear-avoidance, where patients need to relax and let go. Type 2 involves directional sensitivity and poor spinal control, with people repeatedly staying in flexion, extension, or rotation patterns, often moving more through the back than the hips. The main focus is lumbar flexion patterns. Typical findings include a lumbar spine that moves more easily than the hips, symptoms aggravated by sitting and forward bending, and relief with standing or walking. Assessment looks at hip versus lumbar contribution during bending, sway-back posture, habitual sitting, hamstring influence, and rocking-back tests. Treatment centers on retraining hip movement, improving lumbar control, modifying sitting and bending habits, and using repeated symptom-modifying exercises. A key message is that biomechanics matter, but only within a broader multidimensional assessment.
