Implementation of evidence-based care for knee and hip osteoarthritis is the main focus here. Exercise and patient education have strong research support, yet only about one in three patients receive them in practice. To address this gap, a pragmatic implementation model is presented with four key parts: broad dissemination of treatment information, clinician certification and training, ready-to-use materials in a digital toolbox, and a registry to track outcomes. The GLA:D program in Denmark is used as a practical example. It trains clinicians through a two-day course, followed by an eight-week patient program combining education and supervised neuromuscular exercise, with follow-up at baseline, 3 months, and 12 months. Results from tens of thousands of participants show reduced pain, better function, improved quality of life and physical activity, lower painkiller use, and less sick leave. The summary also highlights typical patient characteristics, including older age, frequent overweight or obesity, long symptom duration, and common comorbidities. A broader takeaway is that early non-surgical treatment matters, exercise can match painkillers without similar side effects, and structured implementation models may improve care for other chronic conditions as well.
