Episode summary

How to counter nocebo by pointing out what is right with patients, why pain science is an approach rather than an intervention, and how listening and patient-led goals set a therapeutic tone.

Jarod asks clinicians to shift focus from what is wrong with people to what is right, and to coach rather than heal. That means pointing out strengths: youth, good health, strong hips, good range of motion after surgery, or swelling as a sign of an active immune system. Exercise such as riding a recumbent bike can be framed as tapping the body's own pain relief. Imaging findings can be explained as normal age changes, like wrinkles on the inside, often present long before pain. Setting a therapeutic tone means building alliance, avoiding nocebo language and avoiding structuralist pitfalls. Pain science is not something done to patients in one session; it is a shift in the whole approach, built on careful language, good analogies, effective listening and understanding pain as a protective output, not an input. Following a 2016 paper on listening as therapy, Jarod suggests letting patients tell their story, asking what they need to get better, using their goals, and asking what they would do if their pain were 50% better.
