Episode summary

How manual therapy can be used and explained as a way of calming the nervous system, what peri-pain education means for a whole clinic, and why language is one of the most powerful tools physios have.

Jarod argues that pain neuroscience education does not mean abandoning hands-on care. Evidence supports a little manual therapy, used judiciously, alongside education and exercise, and it can improve short-term outcomes and adherence. The key is how it is explained: not as putting bones back or breaking adhesions, but as comfortable, skilled input that can reduce the brain's perception of danger and so reduce pain. Different techniques act as novel stimuli, which opens a conversation about how practicing pain, like practicing free throws, makes the nervous system better at producing it. Peri-pain education means making pain science part of everything: conversations with patients, families, colleagues, referral sources and front office staff. Other patients overhear explanations and start asking questions. Clinics should remove nocebo cues such as models of bulging discs and torn rotator cuffs. Language and education are among the most powerful and most misused interventions. Wider use of this knowledge could reduce chronic pain, opioid prescriptions, unnecessary imaging and surgery.
