Episode summary

How the Chan classification, the Munich consensus and BAMIC describe a muscle injury, what MRI actually shows, and how BAMIC grades 0 to 3 and classes a, b and c are defined.

This part compares the classification systems most used today. The Chan system adds anatomical layers to the traditional three grades but has no place for primary tendon injury. The Munich consensus separates functional from structural injuries in great detail, yet its grades predict return to play poorly. BAMIC, proposed by Pollock and colleagues in 2014, grades an injury from 0 to 4 on clinical and MRI findings and adds a class for the structure involved: a for myofascial, b for the myotendinous junction, c for intratendinous injuries. Before the grades, Marco covers what MRI is read for, including edema extent, fiber disruption and tendon involvement, and why MRI changes often outlast functional recovery. The second half defines each grade in turn, from grade 0 soreness with a normal scan to grade 3 extensive tears, with the measurements that separate them.
