Episode summary

How a BAMIC grade and class translate into return to full training, why intratendinous injuries need a slower rehabilitation, and how the intramuscular tendon differs from the free tendon.

This part links the classification to rehabilitation, based on the work of Macdonald and colleagues. For each class Marco describes the typical presentation, the healing biology of the tissue involved and an example loading progression: myofascial injuries can return within days, myotendinous junction injuries progress over weeks, and intratendinous injuries progress slowly, with stretch and eccentric load introduced late. He then looks at the intramuscular tendon itself: how its structure, stiffness and nerve supply differ from a free tendon, and why that may explain longer recovery and higher recurrence. Strength and flexibility testing alone cannot reliably tell whether the tendon is involved. The lesson closes on why BAMIC, with good inter-rater reliability and clear links to return to full training, is currently one of the most useful systems for managing hamstring injuries.
