Episode summary

How to balance challenge and support with patients using Daloz's model, a dark room thought experiment for gauging confidence, and practical ways to check what a patient has actually learned.

Daloz's model has four quadrants. Low challenge and low support is stasis, such as passive treatments that teach nothing. Low challenge and high support is comfortable but rarely leads to improvement. High challenge and low support leaves patients feeling stupid and disempowered, often through jargon. High challenge with high support is where growth and self-efficacy happen. Mike draws on Bandura: people confident in their abilities treat difficult tasks as challenges rather than threats. In the dark room thought experiment, patients describe how they would reach a door across a dark room full of obstacles. Their answers show whether they need more support, more challenge or a graded approach. To check learning, ask patients to explain what was discussed in their own words before they leave. Mike also uses a hopeful image of shining a light down a well and guiding patients to climb out rather than pulling them out, and summarizes: guide rather than tell, and give everything meaning.
