Episode summary

Why exercise is one of the best tools for preventing and managing low back pain, how varied the course of back pain is, and which psychological and social factors shape whether exercise helps.

Ben opens by framing prevention of recurrence as part of rehabilitation. Back pain often follows a fluctuating trajectory rather than being simply on or off, and a recent systematic review found exercise alone or combined with education effective for preventing low back pain. Education works best paired with exercise. Specific spinal pathology accounts for under 1% of cases, and only a small share present with clear radiological findings, which leaves the large majority classed as nonspecific. Exercise suits this group well, even though the mechanism of why it works is not known. Ben stresses that exercise is not a magic bullet. Drawing on Peter O'Sullivan's work, Ben lists factors that interact in back pain: stress, sleep, beliefs about the body and about exercise, previous experiences, emotions, work and social factors, and the therapeutic alliance. Ben singles out the patient's expected recovery as an underused prognostic factor, since people who do not expect to recover are less likely to do the things that might help.
