Episode summary

How the cycle of threatening beliefs, vigilance, guarding and avoidance keeps pain going, how sense-making and graded exposure helped Karen out of it, and how clinicians can use exposure to rebuild trust in the body.

Peter summarizes Karen's story as a classic vicious cycle: high pain and distress, fear of the body, avoidance, scans that seem to confirm damage, and symptoms that spread. These responses are logical if you believe you are damaged, but they trap people. Karen describes how hearing David Butler speak about pain neuroscience helped her make sense of her pain, and how exposure work reduced it by around 80% within three months. Peter explains that fear memories cannot be erased, only outcompeted by stronger new memories. He has patients list the things they fear and avoid and work through them, and describes exposing people with neck pain to sudden movement and load. Calming the nervous system first, then disconfirming the feared outcome, makes the learning stick. He also describes a man waiting for a hip replacement whose pain fell after a loading plan, without any challenge to his beliefs. Structure predicts pain poorly; fear, distress and worry are better predictors, and they are modifiable.
