Episode summary

A live demonstration of how Peter opens a consultation, using Karen's own history of years of disabling neck pain to show how questions about meaning, context, fear and impact reveal the story behind the pain.

Peter starts with a simple question, asking Karen to tell her story, and then follows her answers. She describes waking unable to move, feeling as though her neck would snap, and then years of pain marked by a recurring pop, fear of sleeping, hypervigilance around sudden movement and sensitivity to touch. His questions probe what the pop meant to her, what she thought was damaged, and why an MRI showing several disc herniations felt validating yet did not reduce her worry. He notes the contrast that she could still pitch fast-pitch softball while guarding her neck in daily life, and asks whether caring for patients with disabling neck pain shaped her fears. Karen had never connected the two. The interview also maps the impact: pain dictated her work, shopping and travel, made her feel like a burden, and spread to her hand, which frightened her further. Peter reflects each piece back so the pattern becomes clear.
