Episode summary

A Rugby League World Cup pitchside concussion decision made without video replay, what to weigh when making calls under pressure, the long-term effects of concussion, instrumented mouthguards and concussion care in the NHS.

Marwan recounts the 2017 World Cup quarterfinal against Papua New Guinea, when the pitchside video replay was broken. A player who had looked symptom free on the field was withdrawn at half time after a clip shared on social media showed a possible brief loss of consciousness. His lessons: professional registration and player welfare come before the result, stand by a decision once made, and caution must be balanced with pragmatism that comes with experience. About 10% to 15% of people develop persistent vestibular, somatic, cognitive or emotional symptoms, more often with preexisting mental health or cognitive problems. Multiple concussions raise the risk of later problems, with growing evidence of a link to neurodegenerative disease. Instrumented mouthguards with accelerometers record impact count, direction and g-force and may join observation, clinical assessment and the SCAT, with governing bodies setting protocols. In the NHS, emergency departments and GPs mainly exclude structural brain injury, GPs refer persistent symptoms after three months, and specialist services are patchy. Acute soft tissue injury clinics in emergency departments and NHS concussion research offer some hope.
