Episode summary

How eccentric training, cluster sets, blood flow restriction and impact training can be applied in cancer care, and how autoregulation and linear or nonlinear periodization fit training around treatment.

Francesco presents his group's proposal for alternative resistance methods. Eccentric and accentuated eccentric training give high force at low energy cost, cluster sets use short intra-set rests of 15 to 45 seconds to manage fatigue, and blood flow restriction at under 50% 1RM may suit inpatients or people with marked muscle loss, though hypertension and vascular disease are possible contraindications. For each he gives example doses and target groups. Impact training, such as box jumps, heel drops and assisted pogo jumps paired with strength exercises, aims to protect bone and physical function, with risk stratification first. No impact training guidelines exist yet. He explains autoregulation, for example loading more before a chemotherapy cycle and less in treatment weeks, and compares linear periodization for deconditioned patients with nonlinear periodization for trained survivors, using a breast cancer trial as the example. Only 30 to 47% of patients meet the guidelines, so he calls on clinicians to refer patients to exercise as standard practice.
