Episode summary

Dynamic coupling drills for athletic groin pain, from leg change and lateral shuffle streams to skipping and 180 degree cuts, plus calf and soleus capacity work and the criteria for starting running and change of direction.

Dynamic drills target pelvic drop, trunk sway and rotation. The hip lock stream starts with a leg change drill held for 2 to 3 seconds, then adds an overhead press and perturbation with a light weight. The lateral shuffle uses a dowel overhead, then band resistance and reaction to an opponent. Overhead skipping progresses to heavier loads and longer hang time, and 180 degree turns progress from a 5 m plant and reaccelerate to a med ball, varied angles and reactive cues. Andy treats calf capacity as one of the biggest factors in groin pain rehab. Few patients have enough on isokinetic and seated soleus testing, 25 to 30 heel raises is a basic benchmark, and a horizontal rebound test reveals ankle, knee or hip dominant strategies better than a single-leg hop for distance. Calf loading moves from raises through eccentric overload to pogos, drop holds and incline hops. Running starts after a pain-free squeeze test, negative crossover and normal Thomas test. Change of direction follows once acceleration and deceleration are pain free with no next-day soreness.
