Episode summary

How obesity affects health, quality of life, careers and access to care, why fat stored outside adipose tissue drives disease, and what diet, exercise and semaglutide can each achieve, including the cost in muscle and bone.

Obesity affects physical and mental health, quality of life, measurable with the Impact of Weight on Quality of Life questionnaire, career progression and willingness to attend screening. Jennifer reviews effects on muscle and joints, including obesity-related sarcopenia, sleep apnea, breathing, cardiovascular, lymphatic and metabolic function, and fertility. She explains the hypothesis that once fat cells reach their safe storage capacity, fat is stored ectopically in liver, heart and muscle, causing local inflammation and insulin resistance. Liposuction does not improve insulin sensitivity, and safe capacity varies with genetics, being lower in people of Southeast Asian origin than in Caucasians. Diet works through a sustained energy deficit. An EASO working group found exercise training alone gives no more than 2 to 3 kg of weight loss, so exercise should be promoted for health rather than weight. Semaglutide, a weekly GLP-1 receptor agonist, gave about 15% weight loss at 68 weeks versus 2.4% with placebo, but up to 40% of the loss was lean mass, and weight loss by any method can compromise bone.
