Achilles pain provocation testing is taught as a graded sequence, starting with low-load tasks and progressing to higher-load tasks to identify how symptoms respond. The key focus is not calf strength or movement quality, but pain intensity out of 10 and the exact location of pain during each test. The progression begins with double-leg calf raises, then single-leg calf raises, followed by double-leg jumps, single-leg hopping, and, for more athletic individuals, repeated high jumps or forward hopping. The approach should be adapted to the person’s activity level, with sedentary patients starting at lower loads and athletes progressing further if needed. A major distinction is made between mid-substance Achilles tendinopathy and paratendon irritation. Mid-substance tendon pain tends to stay very localized and increases in a dose-dependent way as tendon load rises. In contrast, paratendon pain is usually more diffuse along the tendon sheath and may be more provoked by lower-load, movement-based tasks involving sliding and gliding than by heavier tendon loading. The use of a stethoscope to detect crepitus during ankle movement is also highlighted as a sensitive clue for paratendon involvement.
