Episode summary

What the evidence says about physical activity and cancer risk and survival, how physiotherapy helps prevent and manage pelvic organ prolapse, and where the profession could head next.

Cancer is increasingly managed as a chronic illness, and many cases relate to lifestyle factors such as smoking, diet and obesity. Paul cites Pedersen's 2016 work linking exercise and immunity, in which exercise reduced tumor incidence and growth by over 60% in a mouse model, and a meta-analysis where meeting the WHO activity recommendation was linked to a 7% lower cancer risk, with extra activity bringing smaller further gains. Around nine MET hours a week, roughly three hours of brisk walking, was associated with lower mortality in colorectal and breast cancer patients. In pelvic health, NICE guidelines call for physiotherapy before surgery, and the POPPY trial found 6 to 12 months of pelvic floor exercise reduced prolapse symptoms. Returning to running too soon after childbirth puts high load on a tired pelvic floor. Future directions include teaching health and pain in schools, social and cognitive engagement with older adults, recording exercises on phones, cautious use of virtual reality and working with personal trainers.
